​A medical letter from your doctor stating your disability (not older than 3 months)
​An ITR-DD form completed by a medical professional (not older than 3 months)
​A current bank statement in your name (not older than 1 month)
​A SARS document with your Tax number and SARS stamp (certified, not older than 3 months)
​An up-to-date CV (Curriculum Vitae)
​A copy of your highest qualification e.g. a matric certificate or school report (certified, not older than 3 months)
​A document indicating proof of residence – e.g. a utility bill or store account (certified, not older than 3 months)

2) FOR “UNEMPLOYED” LEARNERSHIP
​A copy of your identity document (certified, not older than 3 months)
​A current bank statement in your name (not older than 1 month)

3) FOR “EMPLOYED” LEARNERSHIP
​Your employment contract or letter from employer on a company letterhead, confirming your employment
